In From the Streets

Duffy Health Center operates In From the Streets, a community-wide program which offers emergency motel shelter, meals, and case management services during extreme cold or hot weather for individuals experiencing homelessness. During the winter, In From the Streets operates when temperatures are below 32 degrees. The program operates year-round on an as-needed basis when there are dangerous weather conditions.

Duffy’s integrated care model, skilled case managers, and timely response keep vulnerable individuals within the safety net of care. The program operates in close collaboration with community partners including human service providers, police departments, and the faith community.
