In From the Streets

Duffy Health Center operates In From the Streets, a community-wide program which offers temporary motel shelter and case management services, including meals, for individuals experiencing homelessness who are medically frail, or need extreme cold or hot weather sheltering. Duffy’s integrated care model, skilled case managers, and timely response keep vulnerable people within the safety net of care. Our close collaboration with community partners – human service providers, police departments and the faith community – reduces duplication of services and has made this initiative a successful regional effort.
