Becoming a patient at Duffy Health Center

If you are interested in becoming a patient at Duffy Health Center, here is what you should know:

Am I eligible for Duffy services?

Duffy Health Center accepts state-sponsored health insurance and Medicare. If you do not currently have insurance, our Connections team can help.

We provide care to individuals who are:

  • Uninsured, have recently become unemployed, or are living with a chronic disability

  • In need of mental health counseling services and/or treatment for a substance use disorder

  • Are homeless, are at risk for homelessness or are unstably housed (for example, are living with friends or relatives)

If you are in need of services, please call (508) 771-9599 to make an appointment with a Connections Case Manager so that they can assess your eligibility.

If you are ready to become a patient at Duffy Health Center, your first step should be to make an appointment with our Connections Case Managers by calling (508) 771-9599.
