Case Manager I - Recovery Support Navigator

The Recovery Support Navigator is responsible for engaging patients in Duffy Health Center’s Medication Assistance, (MAT) Program. This position is part of Duffy’s MAT expansion for this high-risk group. The ideal candidate will have an understanding of street drugs and prescription opioids, at least two years’ experience working with people who suffer from Substance Use Disorders, understanding of Medication Assisted Treatment and experience in peer to peer recovery. Case Manager - Housing First

The Case Manager is responsible for coordinating the care of patients with providers and outside agencies to provide support, guidance, and advocate for the needs of Duffy Health Center clients.  Work includes assessing client needs, developing a service plan, providing interventions, advocating for clients, and coordinating with local organizations to assist clients in obtaining appropriate social services, resources, and assistance.

A candidate for this position must have a Bachelor’s Degree in Human Services, Social Work, Counseling, or Education and at least two (2) years of experience in case management, preferably working with the homeless/at risk population; or an equivalent combination of education and experience.

Associate Director of Clinic Operations

The Associate Director of Clinic Operations is responsible for the supervision of medical floor staff, including nurses and medical assistants, and using data and quality improvement methods to develop strategies supporting practice transformation and to continuously improve team processes and activities.  A candidate for this position must be a licensed or certified health care professional with at least two years of experience in direct patient care in a clinical setting. The most qualified candidate would have a bachelors’ degree in nursing and 3-5 years of relevant experience.
