Case Manager I - Recovery Support Navigator

The Recovery Support Navigator is responsible for engaging patients in Duffy Health Center’s Medication Assistance, (MAT) Program. This position is part of Duffy’s MAT expansion for this high-risk group. The ideal candidate will have an understanding of street drugs and prescription opioids, at least two years’ experience working with people who suffer from Substance Use Disorders, understanding of Medication Assisted Treatment and experience in peer to peer recovery. Individuals with stable recovery are encouraged to apply.

Moms Do Care Recovery Support Navigator

The Moms Do Care Recovery Support Navigator will assist pregnant and postpartum women in finding and maintaining a personalized and enhanced pathway to recovery from drug and/or alcohol addiction. The Peer Mom/Recovery Coach assists staff/team members in the navigation of the individual's journey and serves a unique function within the recovery continuum.

Case Manager - Housing First

The Case Manager is responsible for coordinating the care of patients with providers and outside agencies to provide support, guidance, and advocate for the needs of Duffy Health Center clients.  Work includes assessing client needs, developing a service plan, providing interventions, advocating for clients, and coordinating with local organizations to assist clients in obtaining appropriate social services, resources, and assistance.

A candidate for this position must have a Bachelor’s Degree in Human Services, Social Work, Counseling, or Education and at least two (2) years of experience in case management, preferably working with the homeless/at risk population; or an equivalent combination of education and experience.

Licensed Mental Health Therapist

The Mental Health Therapist will triage, diagnose, treat and conduct brief interventions vis-a-vis warm handoff, in a multidisciplinary team, fast-paced environment, working with those at risk of homelessness.  Must be licensed (LMHC, LCSW, LICSW (preferred)) to provide treatment in MA. Experience providing evidenced based interventions for both addiction and mental health.

Director of Development

The Director of Development is responsible for fundraising and donor relation and also oversees the marketing, community relations and communications functions. Work includes development of donors, management of annual fund-raising appeals, developing and maintaining a positive relationship with the community, and overseeing management of marketing communications, special projects and events.

The Director guides the board and leadership team strategic focus on raising awareness of and generating significant financial support for Duffy Health Center.  The successful candidate will demonstrate highly effective interpersonal, verbal and written communications, and the ability to work with people at all levels.

A candidate for this position must have a bachelor’s or master’s degree in a relevant field and five to seven years of experience in fund-raising.  Experience in developing marketing and communications strategy a plus.

Associate Director of Clinic Operations

The Associate Director of Clinic Operations is responsible for the supervision of medical floor staff, including nurses and medical assistants, and using data and quality improvement methods to develop strategies supporting practice transformation and to continuously improve team processes and activities.  A candidate for this position must be a licensed or certified health care professional with at least two years of experience in direct patient care in a clinical setting. The most qualified candidate would have a bachelors’ degree in nursing and 3-5 years of relevant experience.
