Licensed Mental Health Therapist - Alternative Peer Group

Behavioral Health Innovators, Inc. has partnered with Duffy Health Center to staff Recovery Build - An Alternative Peer Group for teens in need of recovery support. This is an innovative program that combines counseling and social activities. Recovery Build is an evidenced-based model that we plan to scale out across Cape Cod and beyond. This exciting program is a critical resource for teens.  

We are seeking an energetic, youth-focused licensed therapist who is a self-starter to join us in promoting, developing and maintaining this project. The position is a total of 40 hours per week. 32 hours will be with the APG program and 8 hours will be at Duffy Health Center. Candidate must love working with adolescents, is knowledgeable about addiction and youth recovery and works well with youth and their families.

Licensed Mental Health Therapist - Pathways Program

We are currently looking for a part time (20 hours/week) Mental Health Therapist to provide clinical coordination to the Pathways Program.  Based on the Zero Suicide Model, Pathways is a new initiative at Duffy Health Center to identify, assess, provide treatment, clinical coordination and follow up care for people who are at risk for suicide. 

The Mental Health Therapist will use multiple points of contact to provide outreach and engagement to ensure patients have access to and are retained in treatment.  This position requires consultation with partner agencies, data collection and ability to analyze data in conjunction with the Associate Director of Quality Improvement, and strong crisis intervention skills.

Licensed Mental Health Therapist - Embedded

The embedded mental health therapist is responsible for providing mental health and substance abuse treatment to individuals who meet admission criteria for Duffy Health Center. He/she works closely with the medical providers and case management staff; therefore, the clinician must be skilled at working on an interdisciplinary team as well as a behavioral health team. Participation in quality improvement initiatives will be an important function of the position. Must be licensed to provide treatment in MA. This position in full-time, 40 hours/week.

Case Manager - BHCP

The BHCP Case Manager is responsible for coordinating the care of patients with providers and outside agencies to provide support, guidance, and advocate for the needs of Duffy Health Center clients. Work includes assessing client needs, developing a service plan, providing interventions, advocating for clients, coordinating with community groups, state and federal agencies, and local organizations to assist clients in obtaining appropriate social services, resources, and assistance.

A candidate for this position must have a bachelor’s degree in Human Services, Social Work, Counseling, or Education and at least two (2) years of experience in case management, preferably working with the homeless/at risk population; or an equivalent combination of education and experience.  This position in full-time, 40 hours/week.

Case Manager - AIMS

We are currently looking for a full-time AIMS Case Manager to remove barriers to expanded integrated mental health and substance abuse services at Duffy Health Center.  The Case Manager will use multiple points of contact to provide outreach and engagement to ensure existing patients have access to and are retained in treatment, using supportive counseling related to progress with patient developed goals, (e.g. job goals, socialization activities, and accessing resources.

The Case Manager will provide case management services to MAT patients and be responsible for the ongoing review of registries to evaluate gaps in treatment for persons with co-occurring mental health and substance use conditions related to opioids and will also monitor benchmarks related to outcome measures.

Medical Director

The ideal candidate will be an experienced primary care physician. Previous Medical Director experience is not required. Experience in the community health center environment, and/or working with persons experiencing homelessness are desired. Major responsibilities include supervising the provider staff, leading quality of care initiatives, and clinical program development.

Patient Access Representative

The PAR verifies scheduled appointments, insurance information and schedules follow up appointments; enters pertinent patient data and account information; insures data is current and accurate; collects fees as appropriate, explains insurance options and connects patients to staff who can assist with insurance issues. Duties also include medical records document management, general administrative support to medical providers. This position in full-time, 40 hours/week.

Medical Assistant

The Medical Assistant is responsible for preparing patients for appointments with medical providers.  Work includes, checking vital signs, collecting specimens for testing, performing initial health screening/assessments, performing routine tests, updating health and medical information, coordinating prescription call-ins to pharmacies for doctors, scheduling appointments and providing general information and assistance. Billing Specialist

The Billing Specialist is responsible for (under the supervision of senior billing staff) verifying eligibility, determining insurance beginning and end dates, updating changes in the electronic record. Follow-up with staff in order to obtain or confirm information needed to ensure payment for visits. Responsible for charge posting, claim review and generation, remittance posting and account follow up. This position in full-time, 40 hours/week.

Data Assistant
