Duffy Health Center is a non-profit, federally qualified health center located in Hyannis, Massachusetts (Town of Barnstable, Barnstable County). The purpose of Duffy Health Center is to provide caring, comprehensive health care and support services to those who are homeless and at risk of homelessness on Cape Cod, and to be a catalyst for individual empowerment and community solutions to homelessness. We reach our goals by our:

Our medical, mental health and case management professionals offer primary care integrated with mental health and substance abuse treatment and services. Through our integrated model of care, we attend to the physical, mental, spiritual and emotional well-being of each person. For our clients, housing is health. Without housing, health services become a revolving door of care and we connect persons who are homeless to available housing resources.

Case Manager

The Case Manager is responsible for providing support, guidance, and advocate for the needs of Duffy Health Center clients.  Work includes assessing client needs, developing a service plan, providing interventions, advocating for clients, coordinating with community groups, state and federal agencies, and local organizations to assist clients in obtaining appropriate social services, resources, and assistance.

Case Manager I - Recovery Support Navigator

The Recovery Support Navigator is responsible for engaging patients in Duffy Health Center’s Medication Assistance, (MAT) Program. This position is part of Duffy’s MAT expansion for this high-risk group. The ideal candidate will have an understanding of street drugs and prescription opioids, at least two years’ experience working with people who suffer from Substance Use Disorders, understanding of Medication Assisted Treatment and experience in peer to peer recovery. Individuals with stable recovery are encouraged to apply.

Registered Nurse Care Manager

The RN Care Manager will facilitate quality, coordinated care to homeless, high-risk patients.  Responsibilities include developing, implementing, coordinating and evaluating care plans to promote effective education, self-management support and timely health care delivery to Duffy patients. The aim is to achieve optimal quality outcomes. Days, no weekends.

Administrative Assistant

The Administrative Assistant greets and assists patients entering the Health Center by: verifying scheduled appointments, insurance information and scheduling follow up appointments; entering pertinent patient data and account information; insuring data is current and accurate. Duties also include medical records document management, processing prior authorization paperwork and general administrative support to medical providers.

Psychiatric Nurse Practitioner

The Psychiatric APRN establishes goals and objectives for patient care, and assumes direct accountability for outcomes and performance.  Under the clinical supervision of the Psychiatrist, and in accordance with professional standards and protocols, the Psychiatric APRN will coordinate and collaborate with medical and case management providers in our team-based model of care.  Outreach to facilities and the streets is part of the job.
