Licensed Mental Health Therapist-Alternative Peer Group

Behavioral Health Innovators, Inc. has partnered with Duffy Health Center to staff Recovery Build - An Alternative Peer Group for teens in need of recovery support. This is an innovative program that combines counseling and social activities. Recovery Build is an evidenced-based model that we plan to scale out across Cape Cod and beyond. This exciting program is a critical resource for teens.  

We are seeking an energetic, youth-focused licensed therapist who is a self-starter to join us in promoting, developing and maintaining this project. The position is 20 hours per week and will expand as our fundraising efforts support. Candidate must love working with adolescents, is knowledgeable about addiction and youth recovery and works well with youth and their families.

RN Care Manager

The purpose of this position is to provide care management within the scope of licensure for high risk patients as determined by Duffy Health Center. The RN Care Manager facilitates and supports the patient in becoming an active participant in his or her own health care and as a member of the health care team. Responsibilities include developing, implementing, coordinating and evaluating integrated care plans to promote effective education, self-management support and timely health care delivery to Duffy patients. The aim is to achieve optimal quality outcomes.

A candidate for this position must have a Bachelor’s Degree in Nursing and at least three years of experience in a direct patient care in a clinical setting.  Previous case/disease Management experience and working with the homeless/at risk population; or an equivalent combination of education and experience is preferred
