Duffy Health Center is committed to community solutions that will assist our clients in reaching their personal goals, and assist our community in reducing homelessness across Cape Cod. Our local, state, and federal partnerships have resulted in innovative programs that help individuals achieve and maintain health and housing. Community collaborations include:
MA BlueCross BlueShield Foundation provides health benefits enrollment.
Community Health Center Network Behavioral Health Initiative strengthens mental health and addiction treatment, and is led by Duffy Health Center in the region.
Diabetes Disparities Collaborative, a Massachusetts state program, offers chronic disease management.
Housing First, a national model, offers permanent housing to chronically homeless individuals. Duffy Health Center provides ongoing intensive case management services to help clients transition out of homelessness.
Operation in from the Street, a Town of Barnstable regional initiative, subsidizes temporary stays at motels for those individuals at greatest risk.
Opportunities Program, funded by SAMSHA, provides intensive case management services for individuals with substance abuse issues who are homeless or at risk of homelessness.
Pilot House is a sober shelter offering therapeutic and support services to individuals recovering from a history of substance abuse.
Specialty Network for the Uninsured offers free or discounted specialty medical care.
