Opportunities Program

Opportunities Program provides intensive case management services to homeless individuals, and those at risk, who have substance abuse issues. It is funded by the Substance Abuse and Mental Health Services Administration (SAMHSA) through the Center for Substance Abuse Treatment. In 2008, Duffy Health Center was one of 25 organizations across the nation selected to receive $2,000,000 over five years to support this effort.

Supporting the Journey

Working closely with SAMHSA, Duffy Health Center is implementing evidence-based practices to most effectively address the interrelationship among homelessness, mental health and addiction disorders. The funding we receive enables us to:

  • Offer enhanced case management services to 320 local individuals
  • Evaluate and document the efficacy and outcomes of these services by researchers at Brandeis University
  • Participate in national outcomes studies designed to identify successful strategies
  • Hold training seminars to build Duffy’s capacity and enhance staff skills
  • Share training with staff at local agencies

Program Features

Together with the client, an Opportunities case manager develops an individual action plan and provides assistance so each client achieves his/her personal goals related to:

  • Accessing mental health and substance abuse treatment
  • Making positive changes to improve mental health, decrease substance abuse, connect to the community and gain employment
  • Achieving stable housing
