Duffy Health Center is a $6 million non-profit federally qualified health center located in the center of the Village of Hyannis (Town of Barnstable, Barnstable County) on beautiful Cape Cod, Massachusetts. Human Resources Generalist
The Human Resources Generalist is responsible for managing and administering human resources functions for Duffy Health Center. Work includes providing employee relations, benefit administration, Affirmative Action administration, recruitment processes, maintaining legal and confidential records, verifying credentials, interpreting and applying employment laws, and providing information and guidance to Directors and CEO regarding employment issues. Work is performed under the administrative direction of the Chief Financial Officer. The employee plans and carries out the regular work in accordance with standard practices and previous training, with substantial responsibility for determining the sequence and timing of action and substantial independence in planning and organizing work activities, including determining the work methods. The employee is expected to solve most problems of detail or unusual situations by adapting methods or interpreting instructions to resolve the particular problem.
A candidate for this position must have an Associate's or Bachelor's Degree in Human Resources or Psychology, or a related field. Two (2) or more years' experience in human resources required; three (3) or more years of experience in human resources preferred, with some experience in a business or health care environment preferred; or any equivalent combination of education and experience. PHR or SPHR certification preferred.
Part-Time Health Information Data Assistant
(20 hours per week)
The Health Information Data Assistant is responsible for entering, retrieving, compiling and reporting any and all data generated by or through the patient care systems of Duffy Health Center. Work includes entry of patient data, preparing reports, distributing reports, verification of data, investigation of data issues, and maintaining patient data.
Skills required: EXCELLENT Excel and/or RDBM experience, experience with formulas and filtering a must; healthcare background strongly preferred; experience with data entry of medical information (i.e., lab results) strongly preferred; experience working with an electronic health record; ability to work with users at all levels of the organization; excellent communication skills.
RN/NP Care Manager
Duffy Health Center is collaborating with Cape Cod Healthcare's Moms Do Care Project. This is a part time position, up to 24 hours per week.
Duffy is currently looking for a Care Manager who will work collaboratively with the Moms Do Care Site Director (Cape Cod Healthcare), Peer Recovery Coaches and Supervisor (Gandara Center) to assist eligible project candidates in accessing and maintaining Medication Assisted Treatment, prenatal care, primary care, behavioral health and specialty care. The program focus is to provide resources to pregnant women so they are able to pursue self-directed recovery goals from the time of enrollment into Moms Do Care through 6 months post-delivery.
The Care Manager will be responsible for direct or indirect provision of:
- Medical Case Management, advocating for appropriate levels of care across provider networks of primary, specialty and behavioral health.
- Connecting clients to required elements of care with supported referrals.
- Developing and maintaining collaborative relationships with the networks of community partners.
- Preparing and submitting required reports on enrolled participants.
- Collaborating with state-level project partners on project evaluation and infrastructure/capacity building activities.
The ideal candidate will have a nursing background and a specialty in perinatal addiction, Medication Assisted Treatment and/or peer recovery. Individuals with stable recovery are encouraged to apply. Minimum requirements: RN, APRN, CARN preferred.
Case Manager - Recovery Support Navigator
Duffy is seeking a Case Manager - Recovery Support Navigator (RSN) to provide education and outreach to the providers and patients at Cape Cod Health Care in order to engage the patients in Duffy Health Center's Medication Assisted Treatment (MAT) Program. This newly created position is part of Duffy's MAT expansion for this high risk group.
The RSN will be responsible for using creative outreach and engagement strategies with both the staff and patients at Cape Cod Health Care. S/he will develop practices with CCHC staff to identify and refer patients who present with opioid addiction. The RSN will engage the patients and refer them to the MAT Program, develop an individualized recovery plan and regularly review this plan to identify potential for relapse and strategize accordingly. Once a client is enrolled, the RSN will continue to educate and provide support with treatment recommendations. S/he will assist clients and hospital staff to provide linkages and supports to appropriate services. The RSN will coordinate services with Duffy's medical department and other medical and service providers. S/he will facilitate psycho education groups for the clients and provide support, guidance and advocacy. The RSN will complete required clinical documentation according to Duffy policy guidelines and compile data for program evaluation reports. As a member of the MAT treatment team, the RSN will participate in case conferences and help prepare transfer and discharge summaries. The RSN will seek to reengage with individuals who leave the program against medical advice. S/he will participate in grantee, evaluation and relevant community meetings. The RSN will work collaboratively with members of the multi-disciplinary team at Duffy Health Center in order to provide integrated care.
Minimum requirements: Bachelor's degree in social work, psychology or related field and/or LADAC. The ideal candidate will have an understanding of street drugs and prescription opioids, at least two years' experience working with people who suffer from Substance Use Disorders, understanding of Medication Assisted Treatment and experience in peer to peer recovery. Front Desk Administrative Assistant
The person in this position greets and assists patients entering the Health Center by: verifying scheduled appointments and insurance information and scheduling follow-up appointments; entering pertinent patient data and account information; insuring data is current and accurate; collecting fees as appropriate, explaining insurance options and connecting patients to staff who can assist with insurance issues. Duties also include medical records document management, processing prior authorization paperwork and general administrative support to medical providers.
Electronic medical record and Microsoft Office experience required. Position requires a flexible individual who can adapt smoothly to an ever-changing environment. Position requires excellent customer service and organizational skills. Necessary competencies include verbal and written communication skills, problem solving and priority setting.
Some evening hours required weekly. No weekends. Balance of other hours to be determined.
High school diploma/GED required; Associate's or Bachelor's degree preferred. One or more years' experience in a medical office required; five or more years' preferred. Encouraged but not required are bi-lingual skills, both speaking and understanding of Portuguese, Spanish, and English.
