Duffy Health Center is a non-profit, federally qualified health center located in Hyannis, Massachusetts (Town of Barnstable, Barnstable County). The purpose of Duffy Health Center is to provide caring, comprehensive health care and support services to those who are homeless and at risk of homelessness on Cape Cod, and to be a catalyst for individual empowerment and community solutions to homelessness. We reach our goals by our:

Our medical, mental health and case management professionals offer primary care integrated with mental health and substance abuse treatment and services. Through our integrated model of care, we attend to the physical, mental, spiritual and emotional well-being of each person. For our clients, housing is health. Without housing, health services become a revolving door of care and we connect persons who are homeless to available housing resources.

Registered Nurse Care Manager

The RN Care Manager will facilitate quality, coordinated care to homeless, high-risk patients.  Responsibilities include developing, implementing, coordinating and evaluating care plans to promote effective education, self-management support and timely health care delivery to Duffy patients. The aim is to achieve optimal quality outcomes. Days, no weekends.  Comprehensive benefits program available on start date.

Part-Time Child & Adolescent Licensed Mental Health Therapist

Behavioral Health Innovations, Inc. is partnering with Duffy Health Center to launch "Recovery Build: An Alternative Peer Group" for teens in need of recovery support. This is an innovative program that combines counseling and social activities. Recovery Build is an Alternative Peer Group evidenced-based model that we plan to scale out across Cape Cod and beyond. This exciting program is a critical resource for teens.

We are seeking an energetic, youth-focused licensed therapist who is a self-starter to join us in promoting, developing, and maintaining this project. The position is a part-time position, 20 hours per week and will expand to a full-time position as our fundraising efforts support. Candidate must love working with adolescents, be knowledgeable about addiction and youth recovery and work well with youth and their families.
