Duffy Health Center is a $6 million non-profit federally qualified health center located in the center of the Village of Hyannis (Town of Barnstable, Barnstable County) on beautiful Cape Cod, Massachusetts. The purpose of the Duffy Health Center is to provide caring, comprehensive health care and support services to those who are homeless and at risk of homelessness on Cape Cod, and to be a catalyst for individual empowerment and community solutions to homelessness. Without housing, health services become a revolving door of care and we connect persons who are homeless to available housing resources.

Part-Time Health Information Data Assistant
(20 hours per week)
The Health Information Data Assistant is responsible for entering, retrieving, compiling and reporting any and all data generated by or through the patient care systems of Duffy Health Center. Work includes entry of patient data, preparing reports, distributing reports, verification of data, investigation of data issues, and maintaining patient data.
Skills required: EXCELLENT Excel and/or RDBM experience, experience with formulas and filtering a must; healthcare background strongly preferred; experience with data entry of medical information (i.e., lab results) strongly preferred; experience working with an electronic health record; ability to work with users at all levels of the organization; excellent communication skills.
Registered Nurse Care Manager
The RN Care Manager will facilitate quality, coordinated care to homeless, high-risk patients. Responsibilities include developing, implementing, coordinating and evaluating care plans to promote effective education, self-management support and timely health care delivery to Duffy patients. The aim is to achieve optimal quality outcomes. Days, no weekends. Comprehensive benefits program available on start date.
