The mission of the Duffy Health Center is to provide caring, comprehensive health care and support services to those who are homeless and at risk of homelessness on Cape Cod, and to be a catalyst for individual empowerment and community solutions to homelessness.

Duffy uses a holistic model of care – attending to the physical, mental, spiritual, and emotional well-being of each person. Our team of medical, mental health and case management professionals offer primary care integrated with mental health and substance abuse treatment and services.

But we provide more than comprehensive health care. For our clients, housing is health. Without housing, health services become a revolving door of care. We now offer housing through innovative programs like Housing First.

    THE PEOPLE WE SERVE

    Leaving homelessness is often a difficult journey. We offer our clients trust, dignity, respect, and ongoing support for days, months, and even years in their efforts to reach their goals.

    Many of the people we serve have complex medical needs. Last year alone, we provided primary care, mental health, substance abuse services and support to over 2,266 individuals. The total encounter rate for 2008 was over 24,000 visits. This is six times the rate of visits at traditional community health centers, reflecting the intensity of client needs.

    Our clinicians see the daily struggle. We are there in shelters, on the street, in homeless camps, and in our Mobile Health Clinic. Our mission is to help the most vulnerable heal in mind, body, and spirit. We are committed to providing care for those who suffer in the depths–or at the edge–of homelessness. To help them gain hope and start the journey home.

    OUR HISTORY

    Our history is interwoven with our commitment to care for the people who need care the most. For over 14 years, Duffy has provided services at our center in Hyannis, and most recently through the addition of a mobile health clinic. We have been licensed and incorporated since 1997 for primary care and mental health services.

    A free-standing, independent non-profit community health center, the Duffy Health Center has been funded by the federal Health Resources and Services Administration (HRSA) Health Care for the Homeless program since 2002.

    Expanding Mental Health and Substance Abuse Services
    We use the evidence based practice of integrating primary care with mental health and substance abuse services and case management. We are excited about our new programs and opportunities in mental health and substance abuse services. Our goal is to help clients to become empowered, work towards recovery, and become part of the wider community. We are part of a state pilot program that offers an Office-Based Opioid Treatment program, serving almost 100 people. We are expanding our menu of addiction support services at our clinic and in community settings. We are collaborating with other agencies and community health centers to strengthen a continuum of behavioral health care. For more information contact Louise.

    Our Dream – A Home Of Our Own
    The Duffy Health Center has three leased sites – a result of continued expansion to temporary locations as our clients and programs have grown. Lack of space limits us! Our dream is to have our own health center, designed to be a welcoming, therapeutic environment – a stepping stone to health, hope, and home.
